Okay, the lettuce is a given. The dressing can be whatever you want it to be. What of the following four items (yes, you only get four) will you choose to put onto your lettuce?

My family would disown me if they saw me eating a salad without carrots (although I do enjoy a wedge salad from time to time).

I usually put carrots, croutons, bacon, cheese, cherry tomatoes, and broccoli, but since I had to choose only four, I went with cheese, croutons, carrots, and other (for bacon). Has to be the "real" bacon bits...bacos are just nasty.

Cheese, usually cheddar, tomatoes, cukes, green peppers and I really like pineapple pieces or other sweet fruit in it too. I also like raisins and nuts in the salad. A few croutons are nice, but only a few. Limit me to only four choices and I'll take the cheese, tomato, green pepper and pineapple.
And for dressings, I like to mix Thousand Island and Catalina French together. Don't knock it until you try it. I'm about to an iceberg lettuce salad with Vidalia Onion dressing on it. That's good too!

I went with cheese (bleu), olives, tomatoes, nuts (toasted pine nuts).
Also, lettuce isn't a given. I'll take baby spinach or some other leafy green instead please.

Tomatoes, broccoli, cukes and carrots. I usually put in nuts and sweet onion and mushrooms. It really depends on what I have in the fridge to what goes in my salad.

Tomatoes, cucumbers (but it really pained me to have to click next to pickles - blech!), nuts (honeyed almond slivers, sweetened pecans), carrots.
I would go with baby spinach over lettuce. Throw in some dried cranberries and I'm very happy. Salad dressing choice would be olive oil and a flavored balsamic vinegar.

Do pickled onions count as pickles? If so, then I like them but I don't like any other pickles.

For me a salad (assuming we're not dealing with any specialty salads like a Caesar or whatever) is: lettuce, cheddar, croutons, chow mein noodles (the fried ones), bacon bits, sometimes hard-boiled egg, either French or Thousand Island dressing, and if French dressing is used some grated parmesan.
I also like to add some ham or whatever to make it a nice julienne salad.

Put me in the camp with pickle lovers as long as they're not on my salad. I could imagine a few varieties of salads that would benefit from cornichons, but not many.
